Following is Addendum No. 1 for the Bids for supply only of Ice Melt Bid # 2009-40 for Consolidated School District 158. Please acknowledge the addendum on your bid form. The bid due date remains 2:30 p.m. on Monday, August 24, 2009.

ADDENDUM NO. 1

1. The Hold Harmless Agreement form provided in this addendum must be attached to the bid for a complete bid.
2. The General Liability Insurance Requirements shall be revised to a minimum limit of coverage of $1,000,000 per occurrence. The insurance shall be Occurrence based Coverage.
3. Replace the last sentence in the ‘Product Specifications for Ice Melting Compound” section on page 2 of the bid specification with the following sentence. “For a complete bid, the bidder shall provide a copy of the manufacturer’s product literature and the MSDS sheet for the specific manufacturer’s product the bidder intends to provide as a part of the bid.”

By signing this Hold Harmless Agreement, the bidder certifies that to the fullest extent permitted by law, the bidder agrees to defend, pay in behalf of, and hold harmless Consolidated School District 158 and its elected and appointed officials, employees and volunteers and others working in behalf of Consolidated School District 158; against any and all claims, demands, suits, loss, including all costs connected therewith, for any damages which may be asserted, claimed or recovered against of from Consolidated School District 158, its elected and appointed officials, employees, volunteers and others working in behalf of Consolidated School District 158, by reason of personal injury, including bodily injury and death; and/or property damage, including loss of use thereof, which arises out of or is in any way connected or associated with this contract.
